Pathway Grid was founded following a simple experience.
When a family member required community-based healthcare support, essential supplies were arranged through established channels. Shortly afterwards, those supplies were paused, with a delay of several weeks. The same items were sourced independently by the family and delivered the next day. More than a month later, the original pathway resumed.
The issue was not clinical judgement.
It was not procurement policy.
It was coordination.
When physical supplies do not align with care pathways, the burden shifts to patients and frontline teams. Valuable clinical time is absorbed resolving avoidable disruption.
Pathway Grid exists to close that gap.
